Summary of The 36-Hour Day by Nancy L. Mace and Peter V. Rabins | Includes Analysis Preview: The 36-Hour Day by Nancy Mace and Peter Rabins is a detailed self-help guide for people caring for loved ones with Alzheimer’s disease, dementia, and other memory impairments. The burdens on caregivers are immense. They first must have a physician adequately assess their loved one’s condition and help the family come up with a plan for comprehensive and appropriate care. Adjusting to this new way of life can be overwhelming for many caregivers so The 36-Hour Day offers clear strategies for addressing the physical and emotional needs of both dementia patients and those responsible for their care. The term “dementia” describes a group of symptoms, such as mental confusion, loss of memory, and general discombobulation. SpeedReader Summary of The 36 Hour Day The 36-Hour Day was originally published in 1981 and was the first book of its kind on the market. Today, it’s still the definitive guide relied upon by people who are caring for someone with dementia. This bestseller has recently been revised and updated. See what prominent periodicals are saying about it: “Excellent guidance and clear information of a kind that the family needs... The authors offer the realistic advice that sometimes it is better to concede the patient’s frailties than to try to do something about them, and that a compassionate sense of humor often helps.” New York Times “An excellent book for families who are caring for persons with dementia... A book that physicians can confidently recommend to the families of their patients.” Journal of the American Medical Association “An admirably realistic guide to caring for people with Alzheimer’s.” Michael Greenberg, New York Review of Books “An excellent, practical manual for families and professionals involved in the care of persons with progressive illnesses... The book is specific and thought-provoking, and it will be helpful to anyone even remotely involved with an ‘impaired’ person... Highly recommended, especially for public and nursing libraries.” Library Journal

A revolutionary new approach to Alzheimer’s care, focusing on a patient’s strengths to maintain connections with others and the world There currently is no cure for Alzheimer’s disease— though it can be treated. For the last fifteen years, John Zeisel, Ph.D. has spearheaded a movement to treat Alzheimer’s non-pharmacologically by focusing on the mind’s strengths. I’m Still Here is a guidebook to Dr. Zeisel’s treatment ideas, showing the possibility and benefits of connecting with an Alzheimer’s patient through their abilities that don’t diminish with time, such as understanding music, art, facial expressions, and touch. By harnessing these capacities, and by using other strategies, it’s possible to offer the person a quality life with connection to others and to the world. In March 2013, Dr. Zeisel and his work will be the focus of the program airing on public television stations entitled “Hopeful Aging,” bringing his life-changing ideas to a national audience.

During the past several decades, the field of mental health care has expanded greatly. This expansion has been based on greater recognition of the prevalence and treatability of mental disorders, as well as the availability of a variety of forms of effective treatment. Indeed, throughout this period, our field has witnessed the introduction and the wide spread application of specific pharmacological treatments, as well as the development, refinement, and more broadly based availability of behavioral, psychodynamic, and marital and family interventions. The community mental health center system has come into being, and increasing numbers of mental health practitioners from the fields of psychiatry, psychology, social work, nursing, and related professional disciplines have entered clinical practice. In concert with these developments, powerful sociopolitical and socioeconomic forces-including the deinstitutionalization movement of the late 1960s and early 1970s and the cost-containment responses of the 1980s, necessitated by the spiraling cost of health care-have shaped the greatest area of growth in the direction of outpatient services. This is particularly true of the initial assessment and treatment of nonpsychotic mental disorders, which now can often be managed in ambulatory-care settings. Thus, we decided that a handbook focusing on the outpatient treatment of mental disorders would be both timely and useful. When we first began outlining the contents of this book, the third edition of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disor ders (DSM-III) was in its fourth year of use.

Out of Practice

Primary care medicine, as we know and remember it, is in crisis. While policymakers, government administrators, and the health insurance industry pay lip service to the personal relationship between physician and patient, dissatisfaction and disaffection run rampant among primary care doctors, and medical students steer clear in order to pursue more lucrative specialties. Patients feel helpless, well aware that they are losing a valued close connection as health care steadily becomes more transactional than relational. The thin-margin efficiency, rapid pace, and high volume demanded by the new health care economics do not work for primary care, an inherently slower, more personal, and uniquely tailored service. In Out of Practice, Dr. Frederick Barken juxtaposes his personal experience with the latest research on the transformations in the medical field. He offers a cool critique of the "market model of medicine" while vividly illustrating how the seemingly inexorable trend toward specialization in the last few decades has shifted emphasis away from what was once the foundation of medical practice. Dr. Barken addresses the complexities of modern practice-overuse of diagnostic studies, fragmentation of care, increasing reliance on an array of prescription drugs, and the practice of defensive medicine. He shows how changes in medicine, the family, and society have left physicians to deal with a wide range of geriatric issues, from limited mobility to dementia, that are not addressed by health care policy and are not entirely amenable to a physician's prescription. Indeed, Dr. Barken contends, the very survival of primary care is in jeopardy at a time when its practitioners are needed more than ever. Illustrated with case studies gleaned from more than twenty years in private practice and data from a wide range of sources, Out of Practice is more than a jeremiad about a broken system. Throughout, Dr. Barken offers cogent suggestions for policymakers and practitioners alike, making clear that as valuable as the latest drug or medical device may be, a successful health care system depends just as much on the doctor-patient relationship embodied by primary care medicine.
